Care Coordination Support

To help patients who are stuck, overwhelmed, or unable to get services started.

• Help initiate home health referrals (as appropriate)

• Assist with appointment scheduling support

• Follow up on delays or missing steps

• Provide check -ins during the care transition

• Help connect to resources (transportation, meds, etc)
